INFORMATION WE COLLECT ABOUT YOU AND HOW WE COLLECT IT
When you use the Services, you and/or your employees, independent contractors, staff members, and/or donors (collectively, the “Authorized Users”) may provide My Well with two main types of information: (i) personally identifiable information that the Authorized Users knowingly choose to disclose, which is collected on an individual basis, and (ii) general user information that does not contain personally identifiable information, which is collected on an aggregate basis as the Authorized Users use the Services. Examples of the foregoing may include, without limitation, first name, last name, e-mail address and IP address and further described herein. We collect information from you when you (a) use and/or access the Services, (b) open and/or respond to our emails, (c) contact My Well, (d) visit any page online that displays our content, (e) submit or otherwise process donations through the Services, and/or (f) provide information to any of our vendors. Please note, in the event an Authorized User submits a donation via the Services, the Authorized User acknowledges and agrees that by engaging in such transaction the Authorized User may disclose information that may make such Authorized User personally identifiable and such Authorized Users are responsible for the content such Authorized Users provide. In such case, My Well is considered a processor and does not control the personal data of its Authorized Users for other purposes or by other means than instructed by the controller of the personal data. Personally Identifiable Information You Choose to Provide
Personal Data We Collect
When you access or use the Services, we may automatically collect information about you, including:
Usage Details and Device Data. When you download, access or use the Services, they may automatically collect: (i) usage details (including traffic data, location data, communication data and the resources you access and use through the Services); (ii) device information (including IP address, operating system, browser type, hardware ID and other device identifiers, mobile network information, domain, other system settings, language your system uses, time zone of your device and the device's telephone number); (iii) stored information (including metadata and other information associated with the files stored on your device, such as photographs, audio and video clips, personal contacts and address book information); and (iv) location information (including real-time information about the location of your device). We also may use automatic data collection technology to gather information about your activities over time and across third party websites, apps or other online services (behavioral tracking). Please note that we will not access any of your stored information (including photos, personal contacts and address book information), as well as your location information, unless you have specifically opted-in to allowing us such access. The collection of information helps us to make your online experience with us more enjoyable, by allowing us to tailor your online experiences to you while using the Services.
Aggregate. We may collect non-identifying and statistical information about the use of the Services, such as how many visitors visit a specific page, how long they stay on that page and which links, if any, they click on. This information represents a generic overview of our users, including their collective habits. Information collected in the aggregate is not associated with you as an individual. We may share user information in the aggregate with third parties.
Analytics. My Well uses analytic software to gather statistics and usage trends for product and service improvement purposes. Our Services may record data from your phone, tablet, or computer such as how frequently you use the Services, what actions you take and performance data.
Other Tracking Technologies. We may supplement information you provide to us with information from other sources, such as information to validate and/or update your address and/or other demographic information. This information is used to maintain the accuracy of information on the Services and for internal analysis. We may also use clear gifs, pixel tags and web beacons, which are tiny graphic images placed on website pages and/or in our emails that allow us to determine whether you have performed specific actions and are further used to track online movements of our users. In contrast to Cookies, which are stored on your computer’s hard drive, clear gifs are embedded invisibly on web pages. We do not tie the information gathered by clear gifs to your personally identifiable information.
Do Not Track Signals. ‘Do Not Track’ signals are signals sent through a browser informing us that you do not want to be tracked. Because these signals are not universally supported by all browsers and sites, we do not currently assign any meaning to potential ‘Do Not Track’ signals.
Cookies. The technologies we use for automatic information collection may include cookies (including mobile cookies) or web beacons. “Cookies” means the small files sent to and stored on the computer or device you use to access and/or use the Services. Cookies are categorized by how long they are stored and their function. The Cookies My Well uses are as follows:
- Persistent Cookies: Persistent Cookies remain on a visitor’s device for a set period of time specified in the Cookie. They are activated each time that the visitor visits the Services that created that particular Cookie.
- Session Cookies: Session Cookies are temporary and deleted from your computer when you close your web browser. They allow My Well to link the actions of a user while using the Services.
- Strictly Necessary Cookies: Strictly necessary Cookies are essential to navigate around the Services and to use its features.
- Performance Cookies: Performance Cookies collect anonymous data for statistical purposes on how users use the Services; they don’t contain personal information and are used to improve the user experience.
- Functionality Cookies: Functionality Cookies allow My Well to operate the Services in accordance with your choices, such as ‘remembering’ you in between visits.
Third Party Services. To support the services we provide to you, we may use services hosted by third parties. These services may collect information sent by a browser as part of a web page request, including ‘Internet Protocol’ addresses, browser software, and/or clickstream patterns. If such third party services collect information, they do so anonymously without identifying individual visitors. However, we may link the information we record using tracking technology to personal information we collect. While we do not include any third-party advertisements through the Services, when you access and/or use the Services, or access such third party’s content, certain third parties, such as advertisers, analytics companies (e.g. ‘Google Analytics’), your mobile device manufacturer or your mobile service provider may use automatic information collection technologies to collect information about you, your computer or your device. We have no control over this third-parties collection and use of your information. The information these third parties collect may be associated with your personal information or your online or mobile activities over time and across different websites, apps and other online services. Such third parties may use this information to provide you with interest-based (behavioral) advertising or other targeted content. We do not control these third parties' tracking technologies or how they may be used. If you have any questions about an advertisement or other targeted content, you should contact the responsible provider directly.
HOW WE USE YOUR INFORMATION
Use of Information
- operate, maintain and improve the Services;
- answer questions and respond to your requests;
- give you notices about your My Well account;
- perform analytics and conduct research;
- send you reminders, support and marketing messages;
- carry out our obligations and enforce our rights;
- manage our administration of the Services;
- improve the design and content of the Services;
- notify you when updates are available, and of changes to any materials or services we offer or provide through the Services;
- participate in one of our surveys or other forms of customer research;
- facilitate and process donations;
- analyze programs, products and services we offer; and
- use information for other purposes about which we notify you.
The information collected in the aggregate enables My Well to better understand your use of the Services and to enhance your enjoyment as well as providing a more personalized experience by enabling us to estimate our audience size and usage patterns. We use information we collect about your location to customize your online experience to your current location, including providing you with easier access to organizations that use My Well closest to you. We use information to further store information about your preferences, customize the Services, according to your individual interests, speed up your searches, and recognize you when you use the Services. If you use the Services, you agree to receive certain communications from us including but not limited to the following:
Customer Service. If you elect to provide us with your information while using the Services, (for example, by registering on the Services or e-mailing us,) we may provide you with service related announcements concerning My Well or contact you regarding your requests. For example, all registered users receive a welcome e-mail to confirm their registration.
Credit Card Information. We may use financial information to process payment, enroll you in one of our accounts and/or other related services in which you elect to participate. If you are a My Well account holder and have opted in for us to store a portion of your credit card information, provided that, we will not store full credit card numbers, credit expiration dates or ‘CVV’ numbers on our servers. This information is sent directly to our payment processor and then the bank to complete the transaction. We do store other card information such as name, address, phone number and email address in order to assist with your transactions. We also use this data to provide Authorized Users with a history of their giving transactions once logged into their My Well account.
Special Offers, Newsletters and Updates. We will occasionally send you information on products, special deals, promotions and newsletters. You can sign up for these emails from us at any time. Out of respect for your privacy, you may elect not to receive these types of communications by contacting us or replying to our emails.
Parties with whom we may share your information.
Organizations; Aggregated Sharing. We may share information about you with other companies if you give us permission or direct us to share the information. For example, if you submit a donation to a third party, we will share your information with the associated third party so that they can properly receive your donation. We may also share your information in a way that does not directly identify you as an individual. For example, we may combine information related to feedback provided and other submitted data and share the combined information in a way that does not link your information to you. Aggregate data does not contain any information which can be used to identify or contact you. We will never provide your personal information to third parties for their use in marketing their products or services to you without your consent.
For Collaboration. We may share your information, including when you choose to use certain features in the Services that by their nature support sharing with third parties who you choose. Your name, email address, and any content you choose to share will be shared with such third parties, and such third parties may communicate with you in connection with your use of the Services.
Business Transfers. If we sell or otherwise transfer part or all of My Well or our assets to another organization (e.g., in the course of a transaction such as a merger, acquisition, bankruptcy, dissolution, liquidation, etc.), your information such as name and email address, user content and/or any other information collected through the Services may be among the items sold and/or transferred.
YOUR CHOICES ABOUT OUR COLLECTION, USE AND DISCLOSURE OF YOUR INFORMATION
Depending on the situation, My Well may obtain consent to the collection of personal information in different ways. In the event express consent is required, it will be obtained in writing. Implied consent may be obtained through your use of the Services, or when you approach us to obtain information, inquire about or request Services from us. We respect your privacy and give you an opportunity to opt out of receiving announcements of certain information. Although we think that you benefit from a more personalized experience when we know more about you and what you like, you can limit the information you provide to My Well, and you can limit the communications that My Well sends to you. If you access and/or use the Services and volunteer personally identifiable information, you can opt out of receiving notifications, and you may review, modify, update, and/or delete such personally identifiable information by changing your preferences on the Services or otherwise contacting us at firstname.lastname@example.org. Please be aware that even after your request for a change is processed, My Well may, for a time, retain residual information about you in its backup and/or archival copies of its database. Please be aware that we may still email you non-commercial emails related to your account and your transactions through the Services. Following termination of Services, My Well may retain information (including without limitation your personally identifiable information) for a commercially reasonable time for backup, archival, audit purposes, and/or to comply with legal obligations, resolve disputes and enforce agreements. For example, if we are required by law and/or regulation to maintain information concerning you, we will maintain this information pursuant to our then-current document retention policies. Examples of information that may be retained include the history of donations you made to us. In some cases, if you choose not to provide My Well with requested information, you may not be able to use and/or access all of the Services.
Your California Privacy Rights
Under California Civil Code Sections 1798.83-1798.84, California residents are entitled to receive: (a) information identifying any third-party companies to whom My Well may have disclosed personal information to for direct marketing, within the past year; and (b) a description of the categories of personal information disclosed. To obtain such information, please email your request to email@example.com or write us at My Well Ministry, 9550 W. Higgins Road, 8th Floor, Rosemont, IL 60018 and we will provide a list of categories of personal information disclosed within thirty (30) days of receiving such a request. This request may be made no more than once per calendar year. We reserve the right not to respond to requests submitted in ways other than those specified above.

Individuals Under the Age of 13. The Services are not intended for children under 13 years of age, and we do not knowingly collect personal information from children under 13. If we learn we have collected or received personal information from a child under 13 without verification of parental consent, we will delete that information. If you believe we might have any information from or about a child under 13, please contact us at firstname.lastname@example.org. By using the Services, you represent that you are at least eighteen (18) years old and understand that you must be at least eighteen (18) years old in order to create an account and/or purchase the goods and/or services through the Services.
We implement commercially reasonable measures designed to protect your personal information from accidental loss or unauthorized access, use, alteration or disclosure. We periodically review our information collection, storage and processing practices, including physical security measures, to guard against unauthorized access to systems. However, no data transmission or storage system can be guaranteed to be 100% secure. We cannot warrant the security of your information you transmit to My Well or guarantee that information on the Services may not be accessed, disclosed, altered and/or destroyed by breach of any of our physical, technical and/or managerial safeguards; however, My Well implements and maintains appropriate technical, security and organizational measures to protect personal data against unauthorized or unlawful processing and use, and against accidental loss, damage, destruction, theft or disclosure. Please do your part to help My Well. Remember, the safety and security of your information also depends on you and your use of the Services is at your own risk. We urge you to protect your user information and password and to be careful about giving out information in public areas of the Services, which may be viewed by other users of the Services. Please limit your access to your computer and/or mobile device and/or browser by signing off after you have finished accessing the Services. We are not responsible for the functionality, privacy and/or security measures of any other organization.
